Fonar Sales Central - Owners Terms of Use Agreement

I certify that I am an owner of the FONAR UPRIGHT® Multi-Position™ MRI(s).  I would like to use the marketing, advertising and other promotional and/or informational materials FONAR has made available to me via its Website at www.fonarsalescentral.com/members/owners/.  I acknowledge that FONAR is the exclusive copyright owner of all of these materials and further agree to use all these materials, without exception, including print advertising, broadcast advertising, collateral and other sales materials, in the form they are provided to me by FONAR.  I agree that I will not alter them in any way whatsoever, except to imprint or otherwise affix the name, address, phone number, email address and other contact information for the MRI site(s) I own.  I understand that I may also use the materials exactly as they are provided by FONAR, with contact information going directly to FONAR by way of its 1-888-NEEDMRI telephone number, and inform FONAR of my advertising activities, so that when responses to the advertising in my area go to FONAR at 1-888-NEEDMRI, those patients will be booked at my center(s) by FONAR.  I understand that this service is a courtesy offered to me by FONAR and that there will be no charge to me by FONAR for such help with my advertising.
